About the role
Plymouth Care Leavers Service are seeking three new Personal Advisers (PA’s) to support care experienced young people aged between 16 and 25. Our PA’s build positive relationships with young people to assist them in their approach to leaving care and following their 18th birthday and support them to make a successful transition to independent adulthood.
PA’s are the responsible case holder for care experienced young people, working in the community directly with them. Our PA’s support young people to manage good health, build opportunities for education, training and employment, support them to appropriate accommodation, managing their money and support positive relationships with family and friends. PA’s ensure positive Pathway Planning and regular reviews of Plans with young people. This includes liaising with statutory and voluntary agencies and individuals who are involved in the provision of services.
What We’re Looking For
- Qualification - NVQ Level 3 in Health and Social Care (or equivalent).
- Knowledge of the legal and policy context for care experienced young people.
- 2 years’ experience working with young people within the Social Care sector.
- Excellent communication, decision-making, and partnership skills.
